Family PCS:
Allows immediate family members living in the home, to receive compensation for providing care to their loved ones with special needs. This support includes assistance with personal care, household tasks, and community engagement. We aim to enhance quality of life and foster independence by ensuring compassionate, reliable care tailored to each individual's unique needs.
Our case management services provide comprehensive support to individuals with special needs, ensuring they receive the essential healthcare and resources tailored to their unique requirements. We offer personalized assessments, develop individualized care plans, coordinate medical and social services, and advocate on behalf of our clients. Our goal is to enhance their quality of life by ensuring they have access to the right care at the right time.
Our respite/home based services provide essential relief for caregivers of individuals with special needs, offering a chance to rest and recharge. Our trained professionals deliver high-quality care in a safe and nurturing environment. Caregivers can take time for themselves, confident that their loved ones are well cared for and supported.
The behavior support professional offers day habilitation and in-home support to our individuals with a range of services designed to improve the quality of life and promote positive behavioral outcomes for our members. The services completed are as follows: Functional behavior assessments, behavior support plan development and implementation, applied behavior analysis, crisis intervention, skill development and teaching, data collection and analysis. Behavior support also offers training and support to caregivers and staff as well as collaboration with multidisciplinary teams, advocacy and community integration, and monitoring and ongoing support.
